What is the nursing practice issue you have identified related to the topic you have chosen?

The practice issue I have decided to focus on is reducing <30 days readmission rate in older adults in an acute setting.

 

Fully describe the scope of the practice issue:

The “scope of practice” describes the who, what, where, when, why, and how of nursing practice (ANA pg. 2).  It is used to define the actions, procedures and process of the intervention.

This practice issue would be a cause and effects, starting with addressing the concern with key players of the hospital to give the go ahead to start the process. A survey from the patients and families when readmitted would be the first action to get the idea of whether they had resources and support system available when discharged and also if they felt medically ready prior to being discharge. Safety/risk management concerns- Older adults who might have had surgery or been bedridden for days need to be safe at the hospital and at home.

Clinical practice issue is a concern-improving nurses work environments and staffing to help implement the process can be effective interventions for preventing readmissions.  This can reduce unnecessary costs to Medicare

The increase in patient’s readmissions have been noticed mainly from the older adult population due to complications, not medically stable for discharge, lack of support system, and availability of outpatient resources. Educating staff: Implementing change Power Point Presentation. Studies have shown a lot of readmissions are preventable by individualized discharge planning tailored to each patient can reduce hospital readmission.

The effects of developing personalized discharging planning on patient leaving the hospital and the impact on readmission rate.

The goal of discharge planning is to reduce the hospital length of stay and unplanned readmission to the hospital

 

Summarize (in your own words) the interventions the author(s) suggest to improve patient outcomes.

The interventions suggested were discharge planning, case management, patient education and home services. Educating staff: Implementing change Power Point Presentation.All these were centered on personalized discharge planning for patient while in the hospital, with the aim of improving overall health, managing cost and making sure they are well informed and prepared post discharge.

Summarize the main findings by the authors of your systematic review including the strength of evidence for each main outcome.  Consider the relevance to your project proposal for the Milestone 2 project paper. (If an optional supplemental source is also used, include a statement of relevance to it as well.)

The main finding showed that majority of the interventions used were effective at reducing the risk for readmission and hospital length of stay, when discharge planning is tailored to the patient.

 

Outline evidence-based solutions that you will consider for your project.

One evidence based solution I would consider for my project would be using interventions that were simpler and less complicated. On the article, there was a small reduction in the risk of hospital readmission with the above interventions.

Discuss any limitations to the studies that you believe impacts your ability to utilize the research in your project.

Individualized discharge planning, patient education, care management and home services are things that can be done and made available to the patient without additional cost to the hospital.
